Co-developing reporting suite comprising: simplified implementation picture giving visibility across the ‘vital few’ CCG QIPP projects to reduce acute intervention; a milestone tracker providing leading indicators of progress for these vital few; a set of ‘system measures’ tracking the effects of service developments on activity and progress shifting settings of care out-of-hospital.
Co-developing a data-model beneath the system measures which: combines SUS, SLAM and community data to provide a complete picture of acute activity and the out-of-hospital services which aim to reduce acute demand; tracks delivery of OOH objectives compared with plan.
Working within NWL CSU to embed tool-sets and quality assure data feeds
An agreed process and reporting suite which:
• Drew consensus from stakeholders with conflicting demands.
• Distils a simple overview of implementation progress from a complex set of QIPP programmes across eight CCGs and seven providers.
• Provides at-a-glance indication of benefit delivery in shifting care out of hospital and reducing the overall burden of unscheduled care.
An analytical and reporting toolset was produced which we have supported NWL CSU to embed within their commissioning product portfolio – and is helping engage CSU customers to validate and adopt the outputs.
