“Your Care Connected” is a scheme allowing the sharing of patient records across Birmingham, Sandwell and Solihull. The programme had faced challenges throughout its 18 month lifespan, due in part to the ineffective ways of working and the need for improved team member skills. A particular challenge was alignment of the organisations involved, including legitimate challenges brought to the programme by the Local Medical Council. In addition, legal advice instructed the exclusion of social care and ambulance services, the removal of the data warehouse from the technical design, and the introduction of an untenable ‘opt-in’ model. An effective audit and reporting process was required to evaluate the proposed solution, evidence of success was required to enable expansion in to Phase 1, and a procurement exercise for the full technical implementation needed to be run.
We were commissioned initially to assess the status of the programme and then were subsequently engaged to deliver a programme of change, supporting the recovery of the YCC programme. Our support was delivered in four key stages, shaped around the GE change acceleration programme:
• Programme Audit (Creating a Shared Need & Shaping a Vision)
• Programme Recovery (Mobilising Commitment)
• Proof of Concept Delivery (Making Change Last)
• Ongoing Support and Handover (Monitoring Progress)
We provided a programme management role to lead on programme delivery and ensure engagement with a wide range of stakeholders, creating a clear project plan with tasks, responsibilities and realistic timescales. We worked with the programme team and board to provide a clear definition of scope, outcomes, benefits and measures of success and worked closely with the team to identify training gaps and transfer skills appropriately.
A fully tested data sharing approach was validated over a 3 month period for 80,000 patients, leading to board approval to rollout. Our approach to robust programme management coupled with analysis of figures and application of our procurement expertise delivered cost savings of £600k. When added to our upskilling of the programme team and detailed handover to the programme director, the programme is in a strong and sustainable position to fully deliver on its commitments during the course of 2016.
